The deadline to file a Claim Form is October 5, 2026. To complete the Clam Form online, your Unique ID and Password are required to access the form. The Unique ID and Password are located on the email notice you received.
If you do not have a Unique ID and Password and believe that you are a Settlement Class Member, or would like to request a claim form be mailed to you, please contact the Settlement Administrator, RG/2 Claims Administration LLC at J4VSettlement@rg2claims.com or 844-951-2349.
